When making appointments, inform the receptionist of any insurance plans that need pre-authorization such as VSP, Davis, and EyeMed. Please read the information about VSP and Eyemed on this website. Please tell the receptionist if you are interested in contacts or currently wearing contacts. This is to insure quality time for your contact lens evaluation. Please note we recently added DAVIS VISION CARE and FIRSTLOOK VISIONCARE. Major Health Plans including Tricare and Blue Cross Blue Shield and many others are always welcome. Thank you.

Welcome to our family practice. Park Family Eye Care is proud having served the Fox Valley community since 1977, and our mission is to give the highest level of vision care and compassionately responding to patients' eye care needs. Park Family Eye Care is dedicated to providing outstanding quality eye care with state of the art technology. We are committed to the patient's best interest in thorough comprehensive eye exams, treating eye diseases with prompt care, providing quality eye wear, and fitting any specialty contact lenses. We perform well-vision eye examinations as well as diabetes eye examination with a report to your family physician. We are all certified motor vision specialists for the State of Illinois. Our office is dedicated to providing personal, friendly, professional service. We will always try to answer any questions you might have regarding your eye health or eye wear needs. Our practice was established in 1977 in Fox Valley serving towns such as Aurora, Batavia,Bellwood, Westchester, Geneva, St. Charles,Wasco,Eola,Fox Valley,LaFox,Mooseheart, Wayne, Elgin, North Aurora, Yorkville, Elburn,, Warrenville, Naperville, Bolingbrook,Lisle, West Chicago,Davis, Hinckley, Big Rock, Sugar Grove, Montgomery,Plano, Sandwich,Milwaukee, Fort Meyers, FL, and Oswego. We have completed major reconstruction of our office, changing the facade and expanding our building, with additional high-tech equipment, 5 brand new exam rooms(3 exam lanes plus 2 diagnostics rooms),optical lab, staff conference/break room,bookkeeping, doctors library, and a new contact lens teaching center. Please note our office is wheelchair accessible which includes; front ramp, washrooms,and 3 more exam lanes which can accommodate wheelchairs without switching chairs. Ask about our ONLINE CONTACT LENS REORDERING process. If your data is entered by us, then you can go to: www.ReorderContacts.com and refill your lenses and have them sent to your home at your convenience or Click on the left, Order Contact Lenses. We will always put in your yearly exam contact lens prescription.
Click PATIENT FORMS on the left, and download and complete your history before coming in for eye exam. Please include your Primary Care Doctor's Name, Phone and Address,and if possible his/her email and your email for any communication needed to better care for your eye health concerns.
Please note that our office now has the RPS(Rapid Pathogen Screening) detector test to screen within 10 minutes if a patient is contagious with "pink eye" and is able to go to work or school.
Please note that we accept many major medical plans for eye conditions, such as allergies, bacterial, viral conditions, floaters, dry eye, ocular migraines, focusing problems and many others. Please show us your health insurance card.
